Measuring the Unmeasurable: New UVA Study Sharpens Near‑Death Experience Scales

A new analysis from the University of Virginia compares two leading near-death experience questionnaires, revealing strengths, gaps, and a path to better measurement—while reminding us why the ineffable remains hard to score.
How do you quantify the moment when life hangs by a thread and people report clarity, connection, or a reality more vivid than waking life? Researchers at the University of Virginia School of Medicine have taken a fresh look at that question, not by debating what near-death experiences (NDEs) are, but by asking how we measure what people say about them. In work highlighted by UVA’s Medicine in Motion News, a team led by Marieta Pehlivanova, PhD, and Bruce Greyson, MD, compared two established questionnaires designed to assess NDEs—and uncovered both encouraging overlaps and telling gaps.
The study examined Greyson’s 16‑item Near‑Death Experience Scale—first developed in 1983 and used across decades of research—alongside the newer Near‑Death Experience Content Scale (NDE‑C), introduced in 2020 to address perceived blind spots. Using Rasch modeling on responses from more than 700 experiencers, the team found that both tools are tapping into the same underlying construct. That coherence matters: it suggests the shared heart of NDE reports can be approached with scientific consistency. At the same time, some newer items, including “feeling dead,” did not consistently align with the validated hierarchy of perceptions. And both instruments struggled with response categories meant to capture “intensity” in experiences often described as beyond words.
The authors recommend continued use of the original NDE Scale, largely to preserve continuity with decades of data, while also outlining targeted improvements for both measures. Their findings, published in Consciousness and Cognition, are less about declaring answers and more about strengthening the tools that let us ask better questions. For clinicians and researchers, more rigorous measurement can bring clarity to complex cases, support careful documentation, and inform emerging theories about what might underlie NDEs without presuming a single explanation.

People speak of a boundary crossed, a presence felt, a perspective widened—yet struggle to pin those moments to a scale from zero to three. The UVA team’s results honor that tension. They show that it is possible to respect mystery while refining method, to acknowledge ineffability without abandoning measurement.
